Application Checklist

In order to provide a complete and timely quote for your submission, the following items are required:

• Fully completed acord applications for Property, GL, Equipment, Auto, Workers Compensation & Bumbershoot as applicable
• Fully completed NAMU Marina/Boat Dealer or Yacht Club supplemental application depending on the risk.
• 5 Year currently valued loss runs for all lines quoted
• For new ventures, resumes and experience of owners and general managers
